Hospice RN
• Full-time positions in Keene and Peterborough areas
• Graduate of a State licensed basic nursing program
• Two (2) years of professional nursing experience
• Possession of a current State of New Hampshire Nursing license
• Valid driver’s license
• Two+ years of Nursing case management experience

 

HOSPICE RN ON-CALL WEEKEND BAYLOR
• Provide on-call hospice nursing care in the home for our Keene, Peterborough, and Fall Mountain service areas
• Work Friday 4:30 p.m. through Monday 8 a.m.
• Hospice/Palliative Care and/or Oncology Certified, or demonstrate willingness to obtain certification preferred
• Ability to work independently and collaborate as part of interdisciplinary team

 

PEDIATRIC LNA – EVENINGS
• Variable hour and full-time positions in all service areas
• Must have a minimum of 2 years of LNA experience
• Experience working with pediatric patients preferred

 

MEALS on WHEELS DRIVER – Chesterfield Route
• Graduation from an accredited high school
• Have a driving record which meets agency policy
• Have a current, valid NH driver’s license
• Required to communicate with elderly and handicapped individuals and convey needs of meals participants to appropriate staff.  Agility is needed to get in and out of vehicles and homes while delivering the meals.
• Must be able to work outside in all weather conditions while delivering meals.

• Graduation from an accredited high school

• Have a current, valid NH driver’s license

KITCHEN AIDE – at Harper Acres in Keene
• 2-3 hours per day, Monday -Friday
• Graduation from an accredited high school with concentration in home economics or related field;
• One (1) year of experience in food service or related field is preferred; or
• A combination of education and experience deemed to be the equivalent of the above noted education and experience requirements
• Must be able to read, write and perform simple math functions
• Valid driver’s license

 

HOMECARE RN
• Full-time positions for RNs serving the Keene, Peterborough, and Fall Mountain areas
• Graduate of a State licensed basic nursing program
• Two (2) years of professional nursing experience
• Possession of a current State of New Hampshire Nursing license
• Valid driver’s license

 

HOMECARE RN – WEEKENDS
• Graduate of a state-licensed basic nursing program
• One (1) year professional nursing experience under appropriate nursing supervision; or
a combination of education and experience.
• Valid driver’s license
• NH RN license

 

PHYSICAL THERAPIST
$10,000 Sign on bonus for full-time hire!
• Full-time positions available in Keene
• Variable hour positions also available in all service areas
• Graduate of a program in physical therapy approved by the Council on Medical Education of the American Medical Association
• 1 year of professional physical therapy experience
• Possession of a current State of New Hampshire license to practice as a registered physical therapist
• Valid driver’s license

 

PHYSICAL THERAPIST – WEEKENDS
• Variable hour positions available in all service areas
• Possession of a current State of New Hampshire license to practice as a registered physical therapist
• Valid driver’s license

 

HOME SUPPORT PROVIDER
• Variable hour positions in all service areas
• Providing services to include housekeeping, laundry, meal preparation, running errands, etc. for specific clients in their home
• Previous experience working with persons of advanced age or physical or mental disability who need assistance with ADLs preferred
• Must have successfully completed the Home Support Provider competency test with a score of 70% or better
• Valid driver’s license

 

CUSTOMIZED CARE LNA
• Variable hour positions available in all service areas
• Days, evenings, weekends
• Successful completion of approved LNA training
• Minimum of one year experience of active employment as LNA is desired
• Current registration by the New Hampshire Board of Nursing
• Must have successfully completed the LNA competency test with a score of 80% or better
• Must be available to work a minimum of two (2) weekend shifts per month
• If providing care to hospice clients, must be willing to complete hospice training and/or end-of-life training and pass competencies on an annual basis
• Valid driver’s license
