What Is Geriatric Care Management?
Geriatric Care Management is designed to assist you and your family in creating a workable plan for your long term care needs. Care Managers evaluate all aspects of your personal situation, make recommendations, arrange appropriate services, and with your permission, keep your family members informed.

Because Customized Care is part of Home Healthcare, Hospice and Community Services, the Geriatric Care Management team is able to utilize all the services, resources and expertise of HCS, a Medicare certified, state licensed home health agency.

As part of a non-profit organization, Customized Care strives to make these services affordable for all income levels.

How Geriatric Care Management Works

The Geriatric Care Management process begins with a comprehensive assessment of the your physical, mental, and psycho-social health, your living situation, family supports and financial resources.

From that assessment an individualized plan of care is created and reviewed with you and your family. The care manager provides guidance in anticipating and planning for future needs, assistance in evaluating eligibility for benefits, analysis of financial situation and funding options, assistance with the implementation of the care plan, help if relocation is needed, and referrals to legal or financial professionals.

The Care Manager acts as a liaison between families, physicians and care providers.
