Payment Options
Hospice at HCS services are covered by Medicare, Medicaid and many insurance plans. No one is denied hospice service due to their financial circumstances. Hospice at HCS relies on memorial contributions to make services available to all.
Hospice care can be provided wherever people call home, including long term care facilities and assisted living facilities. Working together with the facility staff and the patient's family and physician, our hospice team provides added clinical services and care plan consultations, in addition to providing expertise in pain and symptom management.

Hospice Nursing Home and Assisted Living Standards
  1. The Plan of Care will be established at the time of admission to the Hospice Program by the resident/family, Hospice RN and facility Unit Manager or her/his designee.

  2. A written schedule of visits by the Hospice Team will be posted at the nurse's station to indicate what team member will be visiting that day which indicates the name of the staff member and the day they will be there. This schedule will be updated as needed.

  3. Each visit by the Hospice Team will be documented and placed in the specific area that the facility has requested.

  4. Weekly Care Plan Meetings with the resident/family, Hospice RN and/or Social Worker and the facility staff will be scheduled.

  5. Medical equipment not covered under the facilities room and board will be covered by the Hospice Program.

  6. General Inpatient residents will be seen daily by a Hospice at HCS RN.

  7. Hospice LNA assistance will be provided to hospice residents.

  8. Hospice Volunteers will be available to hospice residents.

  9. Palliative care treatments to improve the quality of life are available and their appropriateness will be determined by the resident's Primary Care Physician and the Hospice Medical Director. Examples are: Intravenous hydration, removal of fluid from lungs or abdomen.

  10. Dementia/Alzheimer's residents will receive Comfort Care Services designed to reduce agitation and confusion.

  11. Hospice at HCS will provide its own Physical Therapy, Occupational Therapy and Speech Therapy to hospice residents to improve quality of life.

  12. Bereavement Support will be provided to the client/family and facility staff prior to and after the death for one year.

  13. Educational programs will be made available to facility staff.

  14. Consultations for symptom management for non-hospice residents will be also available to facility staff upon request.

  15. Specialized Complementary Therapies will be available to Hospice residents when these are appropriate.

  16. All changes and updates to the Care Plan made by the HCS Hospice RN will be discussed with the Unit Manger/Charge Nurse.

  17. At each visit Hospice at HCS will check-in with the Unit Manager/Charge Nurse and inquire about changes in the resident's condition.

The Hospice at HCS team members are highly trained in providing medical care, pain management and emotional and spiritual support.

Our team of hospice advisors can let you know about support that is available to you and your family. Hospice care is available for those with a life limiting illness, including cancer, end stage heart disease, dementia, debility, lung disease, and end stage kidney disease, where ever home is - at home or in a long term care facility.
