Care Coordination can help.
Determining what your family member, friend or neighbor needs and how to pay for it can be a difficult undertaking. Our Care Coordination staff can answer your questions and discuss services that would be helpful. For a more in depth assessment, our outreach staff will visit with you and your family in the comfort and privacy of your own home and outline service options that are available from Home Healthcare, Hospice and Community Services. Outreach staff can also provide information about other resources that are available in the community. There is no charge for your consultation with Care Coordination or for an outreach visit.
