Home Telemonitoring is the collection of vital signs and health status information via a remote system. We monitor blood pressure, pulse, and other vital signs for those who are chronically ill and need assistance in managing their condition.
When vital signs and health status are measured daily with our home telemonitoring program, irregularities can be identified before they become major problems. This increases quality of life, and decreases the number of urgent trips to the emergency room. People using home telemonitoring also benefit by learning how to manage their chronic disease. The increased communication with our health care professional allows for reinforcement of symptom management, medication teaching and self monitoring.
How does telemonitoring work?
The telemonitor is easily installed in the home by our staff, and programmed to ask brief, multiple choice questions about your health status. The telemonitor can also be programmed to provide prompts for medication reminders. Once the data is collected, it is sent via phone line to a secure website, where the data is accessed seven days a week by our telemonitoring nurse for review.
Telemonitors are customized to meet individual needs, based on diagnosis. Telemonitors come with wireless devices used to monitor blood pressure, pulse rate, oxygen saturation, weight, peak flow and blood glucose. Our telemonitoring program complies with all HIPPA regulations for privacy and security.
