Long term planning for life changes can be complex, and finding solutions to challenges necessitated by a health care crisis can be puzzling and frustrating. The social workers at Home Healthcare, Hospice and Community Services are able to help meet the challenges that are brought on by chronic illness or a sudden need for life changes due to an acute illness. Our social work staff provides counseling and advocacy for you and your family.

Outreach staff work closely with our social work and nursing staff to facilitate linkages with other services, including applications for financial assistance.
