When Should I Call      About Home Care?
When you or someone close to you is in the hospital or nursing home, you can ask your doctor or the discharge planner to call Home Healthcare, Hospice and Community Services to help to continue your recovery at home.

If you are at home and feel you need assistance, ask your doctor about home care, or call Care Coordination at 603-352-2253 or 1-800-541-4145 for a home visit to discuss your needs.
