Workplace Wellness Services contracted and paid for by individual businesses and workplaces.
The Employee Heath Risk Assessment Program encourages health and lifestyle changes among employees. The program provides aggregate reports demonstrating effective methods to decrease health care costs.

How does it work?

An HCS Wellness nurse meets with each employee and documents height, weight and blood pressure. Cholesterol and glucose levels are also assessed if desired. The employee completes a questionnaire about their lifestyle.

After the results have been reviewed the nurse again meets with the employee and reviews the confidential results and discusses individual recommendations. The nurse recommends interventions aimed at reducing the individual's heath risks.

Employers also receive a summary report, which includes a concise summary of the groups' health findings, and highlights health practices associated with high risk for disease. The potential economic impact of the group's major health risks is discussed with the employer.
Home Healthcare, Hospice and Community Services (HCS) is Medicare certified, and licensed by the State of New Hampshire. HCS programs are provided to all service eligible individuals without regard to age, race, religion, sex, handicapping conditions, marital or economic status, except where constrained by specialized funding. HCS is a United Way Agency.
