Visiting Nurses assist you to recover at home or to be independent at home – where you want to be. Whether skilled care is needed after a hospitalization or for assistance with a chronic illness, our home care team of visiting nurses, rehabilitation therapists, social workers and home health aides work with your physician to deliver the care you need. You can ask your doctor or the discharge planner to call Home Healthcare, Hospice and Community Services to help to continue your recovery at home.
Home care is not just for older people; home care is people of all ages who need healthcare services at home. We provide not only the skilled care ordered by your physician, but also plan other support services that may be helpful to you and your family.
You or your family members, friends and neighbors can call our Care Coordination nurses to help determine exactly what you might need, as well as determine your insurance coverage for home health services, and other sources of funding that may be available to you for home care. A registered nurse will make a home visit to discuss your care and provide the skilled care that your physician orders.
Our specialized equipment allows you to measure key vital signs at home, which are transmitted to your home telemonitoring nurse for review and analysis. Daily monitoring can detect concerns before they need major intervention, and helps keep you or your loved one at home.
IV Therapies & Wound Care
Our nurses can provide IV therapy and care for your surgical wounds in the comfort of your home.
HCS provides physical, occupational and speech therapies. The focus of our rehabilitation therapies is on recovery, strengthening and home safety, to give you confidence that you can safely be independent at home.
Pediatric Home Care
Our visiting nurses can provide skilled nursing care for your medically-complex child, in the comfort of home.
