Home Healthcare, Hospice and Community Services files a community benefits plan and report of uncompensated care given annually as required by RSA 7:32-e.

In 2008, Home Healthcare, Hospice and Community Services conducted a Community Needs Assessment in conjunction with the following community partners:

The health care needs that were identified in the Community Needs Assessment process, and are addressed in this plan include:
  • Health care affordability
  • Transportation to healthcare resources
  • Availability of health care providers
  • "Navigating the maze" of health care services
Home Healthcare, Hospice and Community Services will support the services listed below to address the needs identified in the community needs assessment.

Prenatal and Well Child Care: The VNA at HCS will continue to strive to improve access to prenatal care for high risk women. Prenatal Clinics in Keene and Peterborough will continue, as well as outreach efforts throughout our service area. Well child efforts assist families with Healthy Kids applications and assisting high risk families in the areas of parenting, connecting with community resources and health education.

The Friendly Bus and City Express will continue transportation services which link seniors and disabled individuals, as well as the public with services and social opportunities. The City Express stops at major medical facilities in Keene 18 times per day, and is actively working with other community partners on medical transportation in rural areas.

Community Clinics and Health Promotion activities are available throughout the year in a variety of community locations. "The Nurse Is In" is an opportunity for people of all ages, with or without insurance coverage, to talk with a nurse, have their blood pressure checked, and participate in other screenings periodically. Age In Motion programs will continue in our service area. This prevention program engages senior citizens in an activity that is positive for their overall health and well-being and provides an opportunity for socialization twice a week.

In addition to the services that are provided to address the needs identified in the Community Needs Assessment, Home Healthcare, Hospice and Community Services provides the following services as community benefits:

The VNA at HCS through its Hospice program offers Bereavement Support Groups and counseling to those in the community who have lost a loved one, whether or not the individual has been involved with the Hospice at HCS program. Support groups offered include Grief Across the Generations, Spirituality and Grief, Adult Daughters Grieving the Loss of their Mothers, and Holiday Grief support groups. Bereavement Drop In times are also available.

The Castle Center for Adult Day Health will continue to provide a supervised setting for adults who need assistance with routine activities during the day. The program assists older adults to remain independent at home and gives family members a respite break.

Elder Care: Older adults will also continue to need assistance with basic activities of daily living in order to maintain independence at home. Homemaker and other supportive services that assist with grocery shopping, laundry and light housekeeping will be provided.

The total cost charity care and the uncompensated cost of programs in the past fiscal year (FY 07-08) was $599,030.00. The anticipated cost of community benefits programs and charity care in the coming year is anticipated to be $622,788.00.
What is the Community Benefits Law?
Under NH RSA 7:32-e, all health care charitable trusts are required to develop community benefits plans to ensure the unique health care needs of a community are met.
Home Healthcare, Hospice and Community Services (HCS) is Medicare certified, and licensed by the State of New Hampshire. HCS programs are provided to all service eligible individuals without regard to age, race, religion, sex, handicapping conditions, marital or economic status, except where constrained by specialized funding. HCS is a United Way Agency.
