HCS was honored to present Care At Home: Understanding Home Healthcare, Hospice & Community Services at Monadnock Community Hospital in March.
The free educational seminar provided an overview of services for older adults, caregivers, and adult children supporting parents. The session covered care options that support independence and peace of mind, including visiting nurse services, home health care, caregiver and family support, community-based programs, and when hospice or palliative care may be helpful. Attendees also learned how HCS collaborates closely with Monadnock Community Hospital and how services are coordinated across home, outpatient, and hospital care.
The seminar was presented by Katie Hart, Senior Director of Operations at HCS.
