HCS is thrilled to announce two new leadership appointments within our organization. Evelyn Erb, LMSW, APHSW-C, has been appointed Associate Director of Social Work and Support Services for Hospice, Palliative & Family Programs, and Elizabeth Moran, RN, will transition into the role of Population Health & Care Continuum Clinical Manager.

Evelyn Erb, LMSW, APHSW-C – Associate Director of Social Work and Support Services for Hospice, Palliative & Family Programs

Since joining our organization 15 years ago, Evelyn has demonstrated exceptional leadership, compassion, and a deep commitment to supporting patients, families, and colleagues. In her role to date , leading our Palliative, Hospice Social Work & Support Programs, she has been instrumental in advancing high-quality person-centered care, strengthening interdisciplinary collaboration, and enhancing the services we provide throughout our communities.

In her new role as Associate Director of Social Work and Support Services for Hospice, Palliative and Family Programs, Evelyn will provide strategic leadership and oversight for social work and supportive services within Hospice, Home Health, Palliative Care, and Family Programs. She will lead the continued development of innovative programs, support our interdisciplinary teams, oversee Spiritual Care, Bereavement, Music Practitioner services as well as oversee the social support team in Healthy Starts, and help ensure patients and families receive compassionate, holistic care throughout their healthcare journey.

Elizabeth Moran, RN – Population Health & Care Continuum Clinical Manager

During her time as Hospice Manager, Elizabeth has provided outstanding leadership and a steadfast commitment to quality patient care. Her dedication to patients, families, and staff has been instrumental in the success of our hospice program.
In her new role, Elizabeth will provide clinical leadership across Home Health, Palliative Care, and Hospice services, focusing on high-risk patient populations, care transitions, staff development, quality outcomes, and population health initiatives. She will help strengthen coordination across the continuum of care while supporting our commitment to patient-centered, value-based care.
