Home Healthcare, Hospice & Community Services is proud to announce the new additional Care Transition Specialist, Nicole Motus, LPN, focusing on the greater -Peterborough area.
Motus will work closely with HCS partner medical providers, facility partners and the community. Her work will include providing Hospice at HCS education to the community and educating family and area staff on quality-of-life options.
Motus brings extensive experience as a Care Transitions and Liaison Specialist from other hospice care providers across the northeast.
