How to make referrals to HCS programs

The kind of care you receive at home is crucial to recovery. Through a combination of evidence-based best practices, a patient-centered team approach, and education for your caregivers, our home health staff members are committed to providing highly skilled care right in your home. To make a referral for Home Health Care:

Referrals must come from a licensed clinical professional (MD, APRN, etc.) and contain orders for Home Care. A complete referral will contain:

Hospice focuses on living life to the fullest while experiencing a terminal illness. Hospice is a holistic approach to end-of-life care, focusing on the quality of life and offering physical, emotional, and spiritual comfort.

To make a referral for Hospice:

Referrals must come from a licensed clinical professional (MD, APRN, etc.) and contain orders for Hospice. A complete referral will contain:

  • Patient Demographics
  • Insurance Information
  • Recent History and Physical
  • Discharge Summary and Visit Notes
  • Medication List
  • MD Orders signed by Attending

The caring staff of the Healthy Starts team at HCS is here to partner with you to build a village of supports to help you thrive in this complicated world. The Healthy Starts team of social workers and home visitors are ready to find the answers you need, promote your strengths, and knock down those barriers that are in the way.

To make a referral to Healthy Starts:

If your patient has diabetes, problems with circulation, or neuropathy, you will need to send in a referral with specific MD orders. Otherwise, a referral is not needed and patients can call  (603) 352-2253 to schedule an appointment.

To make a referral to the HCS Foot Clinic:

The Castle Center offers a little help during the day for folks with medical needs or who need memory care. Socializing, music, art, crafts, meals & snacks are everyday happenings at the Castle Center.

No referral is necessary and there are immediate openings.

Call us at  (603) 352-2253 and ask to speak to someone about the Castle Center.

Meals on Wheels deliveries can help folks stay independent at home by providing good nutrition that is brought to their home. Breakfast bags, weekend meals, and special diets are also available.

Meals on Wheels are currently served in Richmond, Chesterfield, Fitzwilliam, Gilsum, Hinsdale, Jaffrey, Keene, Marlborough, Nelson, Rindge, Sullivan, Swanzey, Troy, Westmoreland, and Winchester. Requests in other Cheshire County towns are considered on a case by case basis.
