How to make referrals to HCS programs

To make a referral for Home Health Care:
- Download and fill out this form
- Fax it back to us with the required documentation at 603-357-2545
Referrals must come from a licensed clinical professional (MD, APRN, etc.) and contain orders for Home Care. A complete referral will contain:
- Patient Demographics
- Insurance Information
- Recent History and Physical
- Discharge Summary and Visit Notes
- Medication List
- Skilled need “Face to Face” Information
- MD Orders signed by Attending
Hospice focuses on living life to the fullest while experiencing a terminal illness. Hospice is a holistic approach to end-of-life care, focusing on the quality of life and offering physical, emotional, and spiritual comfort.
To make a referral for Hospice:

- Patient Demographics
- Insurance Information
- Recent History and Physical
- Discharge Summary (if appropriate), Visit or Office Notes
- Medication List
- Skilled need “Face to Face” Information
- MD Orders for a PCNP consult signed by Attending

The Healthy Starts team of social workers and home visitors are ready to find the answers you need, promote your strengths, and knock down those barriers that are in the way.

If your patient has diabetes, problems with circulation, or neuropathy, you will need to send in a referral with specific MD orders. Otherwise, a referral is not needed and patients can call (603) 352-2253 to schedule an appointment.

Meals on Wheels deliveries can help folks stay independent at home by providing good nutrition that is brought to their home. Breakfast bags, weekend meals, and special diets are also available.
Meals on Wheels are currently served in Richmond, Chesterfield, Fitzwilliam, Gilsum, Hinsdale, Jaffrey, Keene, Marlborough, Nelson, Rindge, Sullivan, Swanzey, Troy, Westmoreland, and Winchester. Requests in other Cheshire County towns are considered on a case by case basis.
