Creating a Partnership
In recent years, increasing numbers of nursing homes and assisted living facilities have recognized the value of offering hospice care to their residents at the end of life. Hospice at HCS – Home Healthcare, Hospice and Community Services has a staff dedicated to working with nursing homes and assisted living facilities. We work as a team with facility staff, providing added clinical services, care plan consultations, and expertise in pain and symptom management.

Home Healthcare, Hospice & Community Services also follows patients as they transition, from home care to hospital to skilled nursing facility, assisted living, or home. Our goal is continuity of care throughout the healthcare continuum.
Bereavement Care for Families
Ongoing support for family members after the loss of a loved one is a critical part of our hospice services. Families and loved ones can receive needed support from our hospice program through counseling, volunteer visits, educational materials, and support groups to help them cope with their loss. Hospice at HCS’s Bereavement Coordinator can also arrange for follow-up care for the facility staff.

The Hospice Philosophy
Hospice focuses on living life to the fullest while experiencing a terminal illness. Hospice is a holistic approach to end-of-life care, focusing on the quality of life and offering physical, emotional, and spiritual comfort.
The hospice goal is to help the patient and family members come to terms with death as a natural part of life. The hospice team of nurses, aides, and other professionals involve the patient and family members in making decisions about their care.
Hospice neither prolongs life nor hastens death. Since cure is no longer the goal, hospice focuses on the quality of life and a dignified death.
Our Mission
The HCS mission is to provide services that enable people to function throughout life at their optimal level of health, well-being, and independence, according to their personal beliefs and choices.
Hospice at HCS is Medicare certified and licensed by the State of NH.
